I read with great interest the volume by Fabio Fantozzi that offers a comprehensive, detailed and documented overview of the complex and extensive topic of dental protection in sport. The topic is of great importance for the physical and psychological integrity of sportspeople, because epidemiological data leave no doubt: a growing trend in the number and severity of injuries, especially in the younger segments of the population, to the point of reaching the relevance of a real public health problem.

As evidence of the importance, even social, of Dental Traumatology, with particular regard to the growth age, also the Ministry of Health spoke on this topic, promoting the divulgation of the Guidelines for prevention and clinical management of dental traumas, prepared from a pool of reliable experts. Prevention is the ‘high road’ especially if we consider that the clinical management of dental traumas is often inadequate. Data from the literature indeed indicate that only a very low percentage of the operators, even less than 5%, is able to perform an appropriate initial treatment, while it is known that the resulting injuries can cause serious damage and their consequences can extend to throughout the life. In the part of the ministerial Guidelines dedicated to primary prevention in sports environment there is a specific reference to protective devices “... it is appropriate that the staff of the sports centers advice mouthguards and / or facial masks depending on the type of sports activity, informing the subject about the long-term benefits of such safety devices (Levin et al., 2012; Çaglar et al., 2010; Çetinbas et al., 2008).”

A recommendation that leaves no doubt about the need for the professional to be updated, to be able to provide the most appropriate advice and clinical care required to the growing number of his patients who practice sports.

In this perspective, this work represents the ideal support; full of checklists, construction protocols, clinical examples, it is very pleasant to read as well as a guide of great practical utility, supported by an iconography of high quality. To the characteristics common to products coming from the hands of qualified technicians, such as effectiveness, efficiency, safety of use and reasonable cost, in this case we should add another one ... some of the works presented in this work are real artworks!.

2014 President of SIDO

The use of mouthguards in sports, considered as non dangerous, does not have any relevance but, data and statistics of numerous hospitals and sports clubs, state and prove the opposite. More and more frequently, in fact, traumas affect the face and especially the area of the masticatory organ: teeth, skeletal structures, such as maxilla and mandible and temporomandibular joint. It is necessary to know that the teeth should be protected and that the means of protection should be sufficiently adequate to the type of practiced sport. The information on the issue is really defective and on the other hand, there is a great lack of knowledge on the part of many athletes and sportsmen of all categories, amateur or professional, about the substantial difference between mouthguards of preformed type and mouthguards of individual custom made type. In this text, particular emphasis is given to the study of comparisons and differences between marketed mouthguards, sold in sporting goods stores and/or on internet and mouthguards individually custom made by dental laboratories based on the prescription of a dentist.

An inappropriate choice of this device, not only does not protect from accidental more or less violent impacts, but worsens the condition of the sport practice and the athlete’s mouth, because the jaw is not ensured by a stable and firm position on closure and the sportsperson, to firmly and steadly hold the mouthguard, has to keep teeth and mouth closed tight.

This situation impairs breathing during shortness of breath and, by necessity, breathing has to occur through the mouth. The flow and the amount of air are so great that, immediate oxygen intake is obtained exclusively by mouth breathing. Despite this, the free fall of the mouthguard impedes conversation with teammates, diverts attention and concentration in the activity that is taking place.

The purpose of this book is to create an informative and practical handbook on dental protection during sports. It is of great aid for dentists, for doctors who are involved with sports medicine, for dental technicians and finally for athletes and sports clubs, so that they can get to know the real risk involved in traumas to the mouth, temporomandibular joints and skull, that at any time endangers those who practice all kinds of sports, especially during workouts when the concentration is lower.

We must not forget that insurance companies do not compensate damages caused to teeth and surrounding structures during sports activities, if at the time of the impact the mouthguard was not worn.
